To apply and submit payment online - visit Ohio Business Gateway then create a user name and password.

Just a few notes to aid in your application...

If you are applying for a corporation, please be sure to give the Charter Number and the Federal ID Number. Your corporation name will appear on the first line of your application.

If your business is not incorporated, the social security numbers of each owner are required.

The business location (#8) should be the actual Warren County location at which the business is taking place. A space is provided for your mailing address in question #7.

When listing the kind of business, please refer to the Sales/Use "NAICS" Codes and choose the most appropriate code for describing your type of business.

Question #9 is to determine the schedule on which you will pay-in your sales tax. If your estimated monthly collection is more than $200.00, you will be required to pay-in every month. If your monthly collection is less than $200.00, you will be required to pay-in semi-annually, or every 6 months.

The application fee is $25.00, and checks should be made payable to the Warren County Treasurer, and mailed to:
